３　市町村等は、支給認定をしたときは、支給認定を受けた障害者又は障害児の保護者（以下「支給認定障害者等」という。）に対し、厚生労働省令で定めるところにより、次条に規定する支給認定の有効期間、前項の規定により定められた指定自立支援医療機関の名称その他の厚生労働省令で定める事項を記載した自立支援医療受給者証（以下「医療受給者証」という。）を交付しなければならない。例文帳に追加
