３　前項の申請をしようとする者は、健康管理手帳交付申請書（様式第七号）に第一項の要件に該当する事実を証する書類（当該書類がない場合には、当該事実についての申立書）（令第二十三条第八号又は第十一号の業務に係る前項の申請をしようとする者にあつては、胸部のエックス線直接撮影又は特殊なエックス線撮影による写真を含む）を添えて、所轄都道府県労働局長（離職の後に第一項の要件に該当する者にあつては、その者の住所を管轄する都道府県労働局長）に提出しなければならない。例文帳に追加
