From old + fashioned. The cocktail (which goes back to at least the early 1800s) got its name in the late 1800s as more complicated cocktails became common and those who preferred simpler drinks began asking for old-fashioned cocktails.

The Old Fashioned is a type of cocktail made by muddling dissolved sugar with bitters then adding alcohol, such as jenever, whiskey or brandy, and a twist of citrus rind. The name references the combination's age: it is possibly the first drink to be called a cocktail. It is traditionally served in a short, round, 8–12 USfl oz (240–350 ml) tumbler-like glass, called an Old Fashioned glass, named after the drink.
