６　第一項第五号に規定する介護予防支援計画とは、居宅において生活を営む要支援者が介護予防その他身体上又は精神上の障害があるために入浴、排せつ、食事等の日常生活における基本的な動作の全部若しくは一部について常時介護を要し、又は日常生活を営むのに支障がある状態の又は悪化の防止に資する保健医療サービス及び福祉サービス（以下この項において「介護予防等」という。）の適切な利用等をすることができるようにするための当該要支援者が利用する介護予防等の種類、内容等を定める計画であつて、介護保険法第百十五条の三十九第一項に規定する地域包括支援センター（第三十四条の二第二項及び第五十四条の二第一項において「地域包括支援センター」という。）の職員のうち同法第八条の二第十八項の厚生労働省令で定める者が作成したものをいう。 - 日本法令外国語訳データベースシステム
