Article 6-2 A person who intends to make a report based on the provisions of Article 6-3, paragraph (1) of the Order shall submit to the Minister of Finance and the minister having jurisdiction over the business, via the Bank of Japan, a written report prepared by using the forms respectively specified in the following items for the categories set forth in said items.
